Prior to your first appointment, please fill out and return (via fax or mail) the Intake Forms in order for Dr. Balzer to understand your medical history and needs prior to your appointment. This will allow for greater efficient use of your time during session. You do not need to print or return the HIPAA Notices of Privacy Practice, this is just for your information.
